Register your Pets

Please fill in the form to the right and acknowledge the information below for each pet you wish to register.

By clicking "Register My Pet" you authorise Chris Humes Veterinary Surgeon to request health records of your pet from their current vet and transfer care.

Please only use this form if you are the legal owner of the animal. 

We will contact you if we have any further questions or to confirm registration using your preferred contact method.

We realise this can be an important decision; if you wish to take a look around the surgery or meet the staff first, then please make an appointment using the details on the Contact page.
